It’s time to re-certify on the Memorial Hermann Health Plan Sales Portal.

Please note that the process has changed from years past. We have simplified the process to allow now you to complete the certification from within the enrollment portal.

Please click on the link below and complete the certification using your current UserID and Password.

You will be able to access the Memorial Hermann Health Plan Sales Portal through the

https://brokers.healthplan.memorialhermann.org website.

Please note if you have not already done so, complete the attached W-9 form and upload it into the portal during the certification process.
